I want to form a company but don’t want my name to appear on company documents

This is not possible. It is a legal requirement for members to be listed on the Memorandum of Association which is viewable by the general public on Companies House. It is also a legal requirement for all directors to be listed with their name, month and year of birth and occupation on Companies House public register.
